CAMP FORMS & DOCUMENTS

CAMPER REGISTRATION FORM

Every camper must complete an online Camper Registration Form.  You can access this form on your Camp Brain account.  Under “View My Submitted Applications”, click on “Summer 2026: Review My Application” and then go to “Forms”.  A Camper Registration Form must be completed for every camper. 

STATE OF CONNECTICUT HEALTH ASSESSMENT RECORD (HAR)

This document constitutes our camp physical and is required for all campers by the State of Connecticut and the Office of Early Childhood (our licensing body).   Camper medicals are good for 2 years and must be valid through the last date the camper will be attending camp.  The HAR must be signed and dated by a licensed medical practitioner (including the list of immunizations).  Please note that Part 1 must be completed and signed and completed by a parent or guardian for the HAR to be accepted.  The HAR must be submitted to the Camp Health Coordinator no later than June 1, 2026 and may be uploaded directly on the Camp Brain portal. 

AUTHORIZATION OF
MEDICATION FORM

This form is used if/when a camper needs to take medication while at camp during the camp day (e.g. for allergies or other medical conditions requiring daily or emergency medication).  This includes all prescription and non-prescription (over-the-counter) medication.  This form must be filled out and signed by a licensed medical practitioner and must be signed by a parent or guardian.  The Authorization of Medication Form must be submitted to the Camp Health Coordinator no later than June 1, 2026 and may be uploaded directly on the Camp Brain portal.  

INDIVIDUAL CARE PLANS
(For campers bringing medication to camp):

In addition to the Authorization of Medication Form, campers bringing medication to camp must have an Individual Care Plan (ICP), signed and completed by a licensed medical practitioner.   

ICPs may also be completed for any camper who requires special or specific care needs that will help them be successful at camp, for example, for campers with dietary restrictions, learning disabilities, behavioral needs and situational needs.  Non-medication ICPs may be completed by a parent/guardian.

CAMP BEHAVIOR POLICY

At Camp Mahackeno and Camp Mini Mack, we believe the best way to prevent potential behavior issues is to provide quality camp programming. If behavior issues arise, we will follow the steps in the Camp Behavior Policy.
